This information is collected for scheduling, invoicing, communication, and service delivery purposes.

ABOUT THESE SERVICES

Heavenly Warrior Yoga & Wellness provides energy-based facilitation, intuitive guidance, body processes, and consciousness education.

Sessions may involve light touch, verbal processes, or intuitive insight and may be delivered in person or remotely.

Experiences vary. No specific results are guaranteed.

These services are educational and experiential in nature and are not medical, psychological, legal, or financial advice.

HEALTH DISCLOSURE

Since only the client is aware of their own physical and psychological well-being and limitations:

I understand that I am responsible for informing the Facilitator of any relevant medical or psychological conditions.

ACKNOWLEDGEMENT & INFORMED CONSENT

Please read carefully before signing.

By signing below, I acknowledge and agree that:

• These services are complementary wellness services and are not medical, psychiatric, psychological, or licensed therapeutic treatment.

• Araceli Guerrero is not acting as a physician, nurse, psychologist, psychotherapist, financial advisor, or licensed healthcare provider.

• No medical advice, diagnosis, or treatment is being provided.

• These services do not diagnose, treat, cure, or prevent any disease or condition.

• I will consult a licensed healthcare provider before making changes to medications or treatment plans.

• I am solely responsible for my health decisions, financial decisions, business decisions, and personal life choices.

• I am choosing to participate voluntarily.

• I understand that results vary and no guarantees have been made.

This agreement applies to today’s session and all future sessions, classes, events, or retreats with Araceli Guerrero, and Heavenly Warrior Yoga & Wellness.

ASSUMPTION OF RISK

I understand that participation in energy-based and personal development sessions may involve emotional, mental, or physical responses.

I voluntarily choose to participate and accept full responsibility for my experience.

CANCELLATION POLICY

• A minimum of 24 hours’ notice is required to cancel or reschedule.

• Appointments cancelled with less than 24 hours’ notice or missed appointments are not eligible for refund or transfer.

• Events and retreats may have separate cancellation policies.

LIABILITY RELEASE

To the fullest extent permitted under applicable laws of Alberta, Canada, the United States, and any jurisdiction in which services are delivered:

I release and hold harmless Araceli Guerrero, Heavenly Warrior Yoga & Wellness, its owners, contractors, assistants, affiliates, and representatives from any and all claims, liabilities, damages, or causes of action arising out of or related to my participation in these services.

This includes, but is not limited to:

• Physical injury

• Emotional distress

• Financial decisions

• Business decisions

• Perceived lack of results

CONFIDENTIALITY

All personal information shared will be kept confidential and used solely for communication and service-related purposes, except as required by law.

Electronic communication carries inherent privacy risks, which I accept.

ACKNOWLEDGEMENT

By signing below, I confirm that:

• I have read and understand this agreement.

• I agree to its terms voluntarily.

• I am at least 18 years of age (or have provided guardian consent below).
